A serious measles outbreak in Pennsylvania, USA, is highlighting how access to healthcare, health beliefs and public health strategies can influence health outcomes. Four people have died from complications of measles, including two newborn babies, as cases have spread through Amish communities.

The Amish are a religious community known for limiting their use of modern technology and having less regular contact with Western healthcare services. However, experts emphasise that low vaccination rates are not necessarily because people are strongly opposed to vaccines. For some, practical barriers such as transport, lack of health insurance and limited contact with healthcare professionals make vaccination less accessible. Estimates suggest that only around a quarter of Amish people in the Lancaster area have received the MMR vaccine. Health departments have therefore taken a community-based approach. Nurses have been visiting people's homes to provide vaccinations, answer questions and build trust. Pop-up vaccination clinics have also been established, with nearly 4,300 MMR vaccines administered by the state since the outbreak began.

This approach demonstrates the importance of understanding the social determinants of health. Simply providing a vaccination service does not guarantee that everyone can access it. Public health professionals may need to adapt services to the cultural, geographical and practical circumstances of different communities.

The outbreak also demonstrates how misinformation can affect health behaviour. Health workers have encountered false claims about vaccine ingredients and safety. Building relationships and providing reliable information may therefore be particularly important when attempting to improve vaccination uptake.
